JOB SUMMARY: To provide individualized nursing care at the place of residence to clients with acute/chronic and complex conditions.

QUALIFICATIONS: Current RN license is required. A BSN is preferred. One or more years of nursing experience is preferred. Medical, surgical, intensive care or inpatient rehabilitation community health nursing experience is preferred. Must have a valid driver?s license, automobile insurance and a good driving record.
