RN Case Manager

Visiting Nurse Association of Linn County is seeking a full-time RN Case Manager. Responsibilities include providing direct patient care in clients’ homes and care coordination. This position includes evening, weekend and holiday on-call rotation. 1 year nursing experience in med-surg field preferred. BSN preferred.
