This employee provides services designed to develop, improve and/or restore the performance of necessary functions; compensate for dysfunctions; and/or minimize debilitation.  The employee exercises professional judgment in the provision of Occupational Therapy services in the home health program.  In selected cases, the Occupational Therapist functions as a Case Manager, using the development of an interdisciplinary plan of care and manages resources to achieve identified outcomes.   Understands and commits to the agency mission.  Performs all duties and responsibilities listed herein, as well as other duties that may be assigned.
